Enrollment & Membership Forms - Panther SCU

The documents below make up the membership packet for Panther SCU.

If you signup for the Educational Student Credit Card, you will be required to have your parent or guardian's signature. Please review the files below and utilize the checklist in order to ensure you have all the necessary documents for membership.

Once completed, put the forms in an envelope and return them to your Student Credit Union branch during regularly scheduled branch hours.

* If you would like a hard copy of the enrollment packet you may request one by going to your Panther SCU office.

STEP 2 - Read and Learn About All That Your Credit Union Offers

STEP 3 - Fill out the Required Forms

Required Forms:

  • Membership Application
  • Debit Card Agreement (If signing up for Checking Account)
  • Credit Card Agreement (If signing up for Student Credit Card)
  • You will need to include $5 minimum deposit in cash/check when opening
  • You will need a Copy of your Student ID and Driver's License (if you have one)
  • If you sign up for Student Credit Card, you will need a Copy of one of your Parent/Guardian Driver's License

STEP 4 - Enclose in Envelope and Turn Into Student Branch

Once you have completed the above and used the checklist to verify you have all required documents you should turn your packet into the Panther SCU staff during regular branch hours.
